get rid of some platform specific stuff
 - use some SDL string functions
 - clean out typedef's for windows specific stuff
 - swap Assert() for SDL_assert()
 - have Int3() trigger SDL breakpoint
 - use cross-platform mul_div()
 - use SDL sized types for fix/ushort/uint/etc.
