implemented MATERIALFLAG_REFRACTION
changed syntax of dp_refract, dp_reflect, added dp_water
moved SHADERPERMUTATION and friends to gl_rmain.c
eliminated SHADERPERMUTATION_MASK by separating out SHADERTYPE_USES
other cleanup
