for now, added the preset cfgs to the video menu. Please replace by real buttons, and add the options from the cfg files in the effects menu too. Effects menu then will need a scrollbar.

very major cleanup of precache code, this patch reduced memory usage
(mostly textures) by 190MB in testing, however the old behavior can be
reenabled by use of the new sv_precache* cvars if any problems arise
added ekg cvar ('engine killing gibs mode' for the ROTT fans)
moved player starting equipment code to level startup

- sv_cheats now gets only read at startup (just like in the engine)
- fix impulse 99 (now give health and armor too, and does not clear item bits any more)

changed transparent color in dieselpower grates again, to be lighter and
more representative of the overall texture color
changed shader for dieselpower grates to use alpha blend instead of
alphafunc
